The Pharmacist eCarePlan (PeCP) is a unique as an interoperability standard that maintains multiple information types and origins: from patient-reported concerns and outcomes, to medication use related details, to intervention data, to laboratory results and vitals, to payment for services. It is the only currently balloted and adopted standard for which multiple utilities, audiences and use result across so many different stakeholders and care team members, but with a focus on optimizing medication use.

Why do we need PeCP?

Why do we need PeCP?

Care planning has become an essential process within the US healthcare system since the majority of morbidity, mortality and health care expenditures are related to chronic disease. Most heatlh care interventions are ongoing, lasting many years or for the remainder of life. With health and payment reform focused on population health management and accountability for patient outcomes, increasing emphasis and resources are being devoted to follow up and monitoring of patient behavior and response to treatment, with frequent assessments and adjustments. Outside of the pharmacy sector, this is commonly called "care planning".
